To replace a lost Jordanian Family Booklet, please submit the following:
- APPLICATION FORM
- 2 photos
- A copy of the applicant’s Residency card, if any
- A copy of the applicant’s valid ID card
- The fee of 11.25 Euros, to be paid in cash
- A copy of the applicant’s Jordanian Family Booklet
- Copies of both the husband’s and wife’s Passports
- A copy of the Marriage Certificate, which must be translated into Arabic and legalized by the Belgian Ministry of Foreign Affairs, and the fee of 11.25 Euros, to be paid in cash
- A letter in Arabic explaining how and where the Family Booklet was lost, a Police Report, which must be translated into Arabic and legalized by the Belgian Ministry of Foreign Affairs, and the fee of 11.25 Euros, to be paid in cash
