Applying for a Jordanian Family Booklet

If applying for a Jordanian Family Booklet for the first time, please submit the following:

  • APPLICATION FORM 
  • 2 photos
  • A copy of the applicant’s Residency card, if any
  • A copy of the applicant’s valid ID card
  • The fee of 4.50 Euros, to be paid in cash
  • The husband’s Family Booklet and the wife’s Family Booklet, in order to register a new booklet under their names
  • Copies of both the husband’s and wife’s Passports
  • Copies of both the husband’s and wife’s Birth Certificates, which must be translated into Arabic and legalized by the Belgian Ministry of Foreign Affairs, and the fee of 11.25 Euros, to be paid in cash
  • A copy of the Marriage Certificate, which must be translated into Arabic and legalized by the Belgian Ministry of Foreign Affairs, and the fee of 11.25 Euros, to be paid in cash
  • For a Jordanian woman married to a non-Jordanian man (to obtain a Family Booklet under her name): a copy of her husband’s passport, ID, and Birth Certificate, which must be translated into Arabic and legalized by the Belgian Ministry of Foreign Affairs, and the fee of 11.25 Euros, to be paid in cash
  • For a Jordanian Muslim woman married to a non-Muslim foreigner: a copy of her husband’s Islam certificate or proof of his conversion to Islam, which must be translated into Arabic and legalized by the Belgian Ministry of Foreign Affairs, and the fee of 11.25 Euros, to be paid in cash
